Institut für Pathologie mit MVZ

Pathologie bedeutet die "Lehre von den Leiden" und beschäftigt sich mit den Ursachen, Entstehung, Verlauf und Auswirkung von Krankheiten auf den Körper. Ärzte für Pathologie werden häufig mit Rechtsmedizinern verwechselt. Dies liegt daran, dass im Englischen der Rechtsmediziner "forensic pathologist" genannt werden und die Übersetzung in vielen Spielfilmen einfach Pathologe lautet. Viele Menschen glauben, dass Pathologen nur Leichenöffnungen durchführen, um Todesursachen festzustellen. Die Haupttätigkeit der Pathologen steht jedoch im Dienst lebender Patienten: Ärztinnen und Ärzte für Pathologie - der Früherkennung und Diagnostik und damit dem Leben verpflichtet!

Obduktionen tragen zur Qualitätssicherung der Behandlung bei!

In der Regel wird eine Obduktion durchgeführt, um die Grund- und Folgeerkrankung sowie die Todesursache genau feststellen zu können. Zusätzlich dient die Obduktion der Ausbildung und Weiterbildung der Ärzte, der Überprüfung von diagnostischen und therapeutischen Methoden und nutzt so für die Zukunft vielen Patienten oder direkt den nächsten Angehörigen bei Feststellung erblicher Leiden.

Gute Pathologen sichern die Qualität Ihrer Behandlung

Die Obduktion dient der Selbstkontrolle der Ärzte, der Ausbildung von Studenten, der Fortbildung junger Ärzte, der Information über die Häufigkeit, Zunahme oder Abnahme von Krankheiten, der Entdeckung von Berufskrankheiten, Umweltkrankheiten und bisher unbekannter Krankheitsbilder, der Früherkennung von gehäuft auftretenden Missbildungen, der Therapiekontrolle (z.B. Überprüfung der Ergebnisse neuer Operationen oder Medikamente), sowie auch der Aufdeckung unnatürlicher Todesursachen.

Ärztinnen und Ärzte für Pathologie helfen Ihnen bei der Krebsvorsorge

Der jährliche Zellabstrich vom Gebärmuttermund, der vom Frauenarzt entnommen wird, untersucht der Pathologe unter dem Mikroskop auf krebsverdächtige Zellen. Nur so kann der Frauenarzt seine weitere Behandlung planen. Solche Krebsvorstufen werden vielfach durch eine Ausschabung oder eine keilförmige Ausschneidung des Gebärmutterhalses entfernt. Anschließend untersucht der Pathologe wie fortgeschritten die krebsartigen Veränderungen sind und ob sie vollständig entfernt wurden.

  • Wird die Diagnose von Krebsvorstufen bei gleichzeitig normalen Schnitträndern gestellt, kann die Gebärmutter erhalten werden - sehr wichtig, wenn es um eine Frau mit Kinderwunsch geht!

Thema Krebsvorsorge: Pathologie hilft Leben retten

  • Krebsvorsorge ist ohne den geschulten Blick des Fachmannes nicht mehr vorstellbar. Auch wenn er im Hintergrund arbeitet: Ihr Pathologe ist ein verlässlicher Wächter über ihre Gesundheit

Ärztinnen und Ärzte für Pathologie spielen schon vor der Behandlung eine wichtige Rolle Nach einer Magenspiegelung zum Beispiel untersucht der Pathologe die aus veränderten Bereichen der Schleimhaut gewonnenen winzigen Gewebsproben unter seinem Mikroskop. Wenn er die Diagnose Magenschleimhautentzündung (Gastritis) mit gutartigem Magengeschwür stellt, was zumeist Folge einer Infektion mit Helicobacter pylori-Bakterien ist, werden Sie anschließend mit Antibiotika in Tablettenform behandelt. Lautet die Diagnose Magenkrebs, so ist eine endoskopische oder chirurgische Entfernung des Krebses zur Heilung notwendig.

Bereits vor der Behandlung werden die Weichen gestellt

  • Die Diagnose des Pathologen bedeutet in vielen Fällen eine wichtige Weichenstellung für die Auswahl des Therapieverfahrens durch den behandelnden Arzt.
  • Es ist der Pathologe, der das Ausmaß der Operation wesentlich mitbestimmt: Je nach Bösartigkeit und Ausdehnung eines Tumors wird individuell vorgegangen. 

Ärztinnen und Ärzte für Pathologie sind auch während der Behandlung bei Ihnen! Während einer Punktion oder Operation der Brustdrüse und aller anderen Organe beurteilt der Pathologe sofort die gewonnenen Gewebsproben in einem mikroskopischen Schnellverfahren. Stellt der Pathologe die Gutartigkeit der Gewebsprobe fest, ist die Operation damit auch bereits beendet. Lautet die Diagnose "Krebs", werden zusätzlich die Ränder des Operationspräparates untersucht, um zu prüfen, ob dieser Krebs im Gesunden entfernt worden ist. Nur so ist es heute möglich, nahezu 2/3 aller Brustkrebspatientinnen brusterhaltend zu operieren!

  • Während einer Operation ist der Pathologe Mitglied des Teams, das Sie betreut. In wenigen Minuten stellt er an der Gewebsprobe wichtige Diagnosen. Der Pathologe als Lotse der Therapie: mehr Lebensqualität!
  • Die intraoperative Schnellschnittdiagnostik entscheidet über das weitere Vorgehen: kann die Operation beendet oder muss sie erweitert werden? Ärztinnen und Ärzte für Pathologie brauchen Sie nach der Behandlung gleich wieder!

Unsichtbares wird sichtbar: im Mikroskop des Pathologen

Beispiel Dickdarm-Operation: Nach Entfernung eines Tumors werden dessen Ausdehnung, der Grad der Bösartigkeit, die Schnittränder und die Frage untersucht, wie weit der Tumor in umgebendes Gewebe eingewachsen ist. Besonders wichtig ist die Untersuchung aller mitentfernten Lymphknoten mit der Frage, ob hier schon Tumorabsiedlungen ("Metastasen") nachzuweisen sind. Dazu fertigen medizinisch-technische Assistentinnen von dem Operationspräparat hauchdünne Schnitte an, die nach Färbung des Gewebes vom Pathologen mit Hilfe des Mikroskops begutachtet werden. Anhand dieser Schnitte kann der Pathologe diese Fragen über das Stadium der Krebserkrankung beantworten und somit die Weichen für das weitere diagnostische und therapeutische Vorgehen bestimmen. Ähnliches gilt für alle anderen Krebsoperationen, z.B. bei Schilddrüsen-, Lungen-, Kehlkopf-, Brust-, Magen-, Haut-, Harnblasen-, Prostata-, Hoden-, Eierstock- und anderen Krebserkrankungen. Der Befund des Pathologen liefert eine wesentliche Entscheidungshilfe für die weitere Therapie.

  • Die Diagnostik des Pathologen am Operationspräparat entscheidet darüber, ob die operative Behandlung ausreichend war, oder ob eine zusätzliche Strahlen- und/ oder Chemotherapie erforderlich ist.

Ärztinnen und Ärzte für Pathologie sind auch neuen Krankheiten auf der Spur

  • Viele bis dahin unklare Krankheiten sind durch Pathologen aufgeklärt worden.
  • Forschung durch Pathologen erbringt bessere Vorsorge, bessere Diagnostik und bessere Behandlung.

Befunderstellung

Biopsien und die meisten Operationspräparate werden am Tag nach dem Eingang bei uns befundet. Bei sehr großen Operationspräparaten sowie diagnostisch problematischen Fällen, insbesondere wenn immunhistochemische Untersuchungen oder eine Entkalkung notwendig sind, kann sich die Befunderstellung um einige Tage hinauszögern. In den meisten Fällen erhalten Sie dann aber zuvor einen vorläufigen Bericht.

Befundübermittlung

Die Befunde gehen Ihnen auf dem Postwege zu oder können digital, unter automatischer Verknüpfung mit der Krankenakte des betreffenden Patienten -ähnlich den Laborwerten- übermittelt werden. Auf Wunsch des Einsenders können die Befunde zusätzlich gefaxt werden. Der Einsender übernimmt dabei die datenschutzrechtliche Verantwortung.

Weiterbildungsplan für Assistenten am Institut für Pathologie

24 Monate Basis-Weiterbildung

  • Unter Aufsicht und Anleitung von Fachärzten bzw. fortgeschrittenen Assistenten sog. kleine Chirurgie mit z. B. Präparation Appendix vermiformis, Gallenblase, Hüftkopf, Magen-Darm-Resektat, Präparate aus dem HNO-Bereich, Strumaresektat, Weichteilexzidate wie z. B. Synovia und Bursa, gynäkologische Pathologie, Uterus, Schilddrüse, Amputate usw.
  • Im 1. Monat Hospitieren bei Sektionen mit schrittweiser Heranführung an die Organpaketpräparation. Im Laufe des 2. Monats unter Anleitung eines Facharztes bzw. fortgeschrittenen Assistenzarztes Durchführung der ersten eigenen Sektion, ab dem 3. Monat selbständige Präparation der Organpakete und Abnahme der Obduktion durch einen Oberarzt. Nach 6 Monaten Präparation von endoskopischen Resektaten aus dem Gastrointestinaltrakt.
  • Bei vollständigem Beherrschen sämtlicher Präparate der kleinen Chirurgie Heranführung an Präparate mit schwieriger bzw. aufwändigerer Präparation wie z. B. Kolonpolypen, Präparate aus der Herzchirurgie und Beginn mit gynäkologischer Zytologie unter Anleitung eines Facharztes und einer Zytologie-MTA.

48 Monate Weiterbildung zum Facharzt für Pathologie

  • Weiterer Ausbau und Vertiefen der vorhergehenden Inhalte sowie Erlernen der Präparation von Präparaten aus dem urologischen Trakt (Niere, Prostata und Lungentumoren, Pankreastumoren, Leberstanzen)
  • Weitgehend selbständiges Befunden von Biopsien und Polypen aus dem Gastrointestinaltrakt unter Kontrolle eines Fach- bzw. Oberarztes
  • Punktionszytologie, Mammastanzen, Haut- und Prostatastanzen, Lymphknoten, Knochenmark, Hospitation Molekularpathologie (Internationale Akademie für Pathologie und verschiedene Institute)
  • Wissenschaftlich begründete Gutachten zu Sektionsfällen werden ab dem 1. Weiterbildungsjahr von den Assistenten durchgeführt.
  • Molekularpathologische Inhalte werden zusammen mit Herrn Prof. Hartmann, Universität Erlangen, vermittelt.
